MANDATORY PHYSICAL EXAM & IMMUNIZATION
Registration WILL NOT be complete until all Sarah Behn Campers submit a copy of their most recent physical exam and immunization records to our office with your application, or as soon as possible thereafter. In order to participate at the camp, this physical must have taken place within the last two years. Due to the Board of Health regulations, each camper must submit an updated copy of most recent physical & immunization record regardless of previous attendance at camp. Campers may utilize the Sarah Behn Basketball Camp Physical Form, or a comparable form received from their physician's office.

Sarah Behn Basketball Camp complies with regulations of Massachusetts Boards of Health, and is licensed by the local Boards of Health.

You will be required to complete and sign a medical release form prior to attending any sessions. Forms are available by mail request or clicking this medical release.

No personal checks accepted on Registration Day at Mount Ida College, The Governor's Academy and Bridgewater State University overnight camps. Cashier's checks, money orders or cash only.
